If you are a health care organization, you will need to have the information below to complete your enrollment online.

Provider information

To start, we'll need your organization name, mailing information and taxpayer identification number (TIN).

New user information

To assign multiple users to your Optum Pay account, please provide the contact information for each designated Optum Pay admin.

Bank information

In order to get funds into your preferred account, please include your routing and bank account information.

Financial information

Lastly, have a signed and dated copy of your organization’s federal W-9, along with a voided check or bank letter for each account where payments will be deposited.
