Optum and Saber Healthcare Group have worked closely for the last eight years to deliver care to Saber’s nursing home members. Through their shared vision, they adhere to a member-centered, evidence-based approach that follows members’ goals of care and respects their autonomy.
When Saber began planning its response to the many challenges posed by the COVID-19 pandemic, it looked to its trusted partner Optum for help.
Working together, Optum and Saber ensured that patients who tested positive for COVID-19 or were symptomatic received appropriate care. They also achieved the following:
- Provided remote care to some patients and focused on advanced care planning
- Made face masks for Saber facility staff
- Facilitated telehealth to help nurses connect with patients
- Navigated end-of-life care by providing grief counseling and emotional support to employees and members
